Wegovy dosage chart: 0.25 mg to 7.2 mg HD, plus the pill

Wegovy dosage is set by the FDA at six injection strengths, from 0.25 mg to 7.2 mg once weekly, plus four daily tablet strengths from 1.5 mg to 25 mg. All of it appears in the FDA prescribing information for Wegovy, version posted to DailyMed on June 30, 2026.

The top injection dose, 7.2 mg, is three times the 2.4 mg maintenance dose. The UK regulator's January 2026 approval notice described the 7.2 mg dose as three 2.4 mg shots in a row, which is not how the US label lists it.

Labels move. The label's own recent-changes list shows Dosage and Administration revised in August 2025, December 2025, and March 2026, so a page written before then can be out of date. Everything below is what the label, the Medication Guide, the manufacturer, or a named trial says. Any choice beyond that belongs to your prescriber.

Wegovy dosage chart: the injection schedule week by week

The Wegovy injection schedule starts at 0.25 mg once weekly and rises every 4 weeks through 0.5 mg, 1 mg, and 1.7 mg, with a maintenance dose beginning in week 17. The label's Table 1 applies this one climb to every approved use in adults and to adolescents 12 and older. The label says the slow climb reduces the risk of gastrointestinal side effects.

If a dose is not tolerated during the climb, the label says to consider delaying dosage escalation for 4 weeks. The manufacturer's Wegovy pen page adds that some people stay longer than 4 weeks on one dose, and that a healthcare professional monitors progress at each dose.

Table 1 stops at week 17 and does not list steps between 2.4 mg and 7.2 mg. The 7.2 mg section below covers what the label does say about that step.

Wegovy maintenance doses: 1.7 mg, 2.4 mg, and 7.2 mg

The label names three injection maintenance options for weight reduction in adults: 1.7 mg, 2.4 mg, and up to 7.2 mg. It marks 2.4 mg as the recommended one. The label states different maintenance doses for each approved use, and our Wegovy page has the full medication profile.

For weight reduction and cardiovascular risk, the label tells prescribers to consider treatment response and tolerability when choosing between 1.7 mg and 2.4 mg. Only one weight-loss trial in the label tested 1.7 mg. In Study 6, which enrolled 401 adults in Japan and South Korea, mean weight change at 68 weeks was 9.6% loss on 1.7 mg, 13.2% on 2.4 mg, and 2.1% on placebo.

The label reports no weight-loss trial results for the 0.25 mg, 0.5 mg, or 1 mg strengths as maintenance doses. Those are escalation steps on the chart.

Wegovy 7.2 mg (Wegovy HD): what the US label says

In the US, Wegovy 7.2 mg comes as one single-dose pen called Wegovy HD, holding 7.2 mg in 0.75 mL. Section 3 lists it as its own strength, and Section 16 gives NDC 0169-4572-14 in a carton of four pens. The UK regulator's January 2026 approval notice described 7.2 mg as three 2.4 mg injections given one after another. The US label does not describe that method.

The label says the dosage "may be increased to a maximum dosage of 7.2 mg" for adults who tolerate 2.4 mg for at least 4 weeks and for whom additional weight reduction is clinically indicated. The manufacturer's Wegovy FAQ uses similar wording for adults who want to lose additional weight after at least 4 weeks on 2.4 mg. The label does not give a step schedule between 2.4 mg and 7.2 mg, and it says 7.2 mg has not been established in pediatric patients.

The trial protocol is more specific, though it is a study design and not a label instruction. According to the ClinicalTrials.gov record for STEP UP, the 7.2 mg arm stepped through 0.25, 0.5, 1.0, 1.7, 2.4, and 7.2 mg every fourth week. The record describes this as a 20-week escalation period followed by 52 weeks of maintenance, for 72 weeks in all. The 2.4 mg arm followed the same steps and stopped at 2.4 mg.

The gap between doses in STEP UP was 3.3 percentage points in the label's analysis. The Lancet Diabetes & Endocrinology paper reports slightly different figures, 18.7% on 7.2 mg and 15.6% on 2.4 mg, from a different analysis than the label's. The label also says Wegovy produced less weight reduction in people with type 2 diabetes than in people without it, which matches the lower rows above.

Wegovy pill dosage: strengths, schedule, and how to take it

The Wegovy pill starts at 1.5 mg once daily and steps up every 30 days through 4 mg and 9 mg to a 25 mg maintenance dose that begins on day 91. The tablets share one label, and one DailyMed record, with the injection. The label covers the tablets for weight reduction and cardiovascular risk reduction in adults.

The label's instructions for the tablets are specific. The label says to take one tablet in the morning on an empty stomach with water, up to 4 ounces, and no other liquid. It says to swallow the tablet whole, without splitting, crushing, chewing, or dissolving it. It sets a limit of one tablet per day and a wait of at least 30 minutes before eating, drinking, or taking other oral medicines. Tablets come in bottles of 30. The manufacturer's Wegovy pill page says dose increases happen every 30 days, as tolerated, until 25 mg.

The tablet label handles tolerability differently from the injection label. If a dose is not tolerated during the climb, the tablet label says to consider delaying escalation but gives no length of delay, where the injection label says 4 weeks. If the 25 mg dose is not tolerated, the label says to consider switching to the 1.7 mg injection. The label also says the tablets have not been studied for weight reduction in adults with type 2 diabetes, and that some patients with type 2 diabetes may have subtherapeutic blood levels on the pill.

The label's Section 2.5 covers moving between forms. It describes starting the 25 mg tablet one week after stopping the 2.4 mg injection. The pill's main trial was OASIS 4. In the New England Journal of Medicine report, 205 adults were randomly assigned to 25 mg daily and 102 to placebo, with the main weight result measured at week 64. The label shows mean weight change of 13.6% loss on the pill and 2.4% on placebo. The published paper reports 2.2% for placebo under a different analysis. Gastrointestinal side effects hit 74.0% of pill users and 42.2% of placebo users.

Weight loss at each Wegovy dose: what trials measured

Trials measured weight loss at the end of a maintenance dose, not at each step of the climb, so the label reports no per-step weight-loss number. The table below lists what the label reports for each maintenance dose. These are different trials with different people, so the rows are not head-to-head comparisons.

Missed doses and slowing the climb: what the label says

For a missed Wegovy injection, the label sets a 2-day rule. If the next scheduled dose is more than 2 days (48 hours) away, give the missed injection as soon as possible. If the next dose is less than 2 days away, do not give it, and resume on the regular day of the week.

After 2 or more consecutive missed injections, Section 2.4 says to reinitiate dosage escalation at a lower dosage to reduce the risk of gastrointestinal side effects. The Medication Guide, which is the patient-facing text of the label, tells patients who miss doses for 2 or more consecutive weeks to contact their healthcare provider about how to restart. The label does not name the lower dose.

Section 2.1 and the Medication Guide both say the dose is used once each week on the same day, at any time of day. The Medication Guide adds a rule that Section 2 does not state. A patient may change the day of the week as long as the last dose was given 2 or more days before. Changing the time of day or the injection site needs no dosage change, per Section 2.1.

The label gives no rule for splitting a pen, for combining strengths, or for staying on a lower dose as a plan. It gives no time limit for a delay beyond the 4-week language. Those questions go to the prescriber.

Wegovy pens: strengths, single-dose design, and injection sites

Wegovy single-dose pens are preset to one strength, so the strength printed on the carton is the dose that pen delivers. The Instructions for Use state that the dose is already set on the pen. The manufacturer says each strength has its own color-coded pen and that a pack holds 4 pens of the same dose. The label text does not list the colors, and the Instructions for Use tell patients to check the name and colored label.

Label Section 2.1 says to inject Wegovy under the skin of the abdomen, thigh, or upper arm and to rotate sites with each dose. The same molecule has its own site guide in where to inject Ozempic, and the rotation idea is the same. Before the cap comes off, the label allows single-dose pens to be kept between 46 and 86 degrees Fahrenheit for up to 28 days, and otherwise says to store them in the refrigerator.

Wegovy dosage vs Ozempic dosage

Wegovy and Ozempic contain the same drug, semaglutide, but the labels set different steps and different ceilings. The Wegovy maximum is 7.2 mg once weekly and the Ozempic maximum is 2 mg. The table compares the two labels, including the Ozempic prescribing information. Ozempic's dosing is built around glycemic control in type 2 diabetes, and Wegovy's around weight reduction, cardiovascular risk, and MASH.

Wegovy dosing in adolescents, older adults, and kidney or liver conditions

Wegovy dosing in adolescents 12 and older follows the same Table 1 climb as adults, with a maintenance dose of 2.4 mg (recommended) or 1.7 mg. The label supports this with a 68-week trial of 201 adolescents. In that trial, BMI fell 16.1% on Wegovy 2.4 mg and rose 0.6% on placebo. The label says 7.2 mg and the tablets have not been established in pediatric patients, and it states that safety and effectiveness are not established under age 12.

Older adults get no separate dose in the label. It reports no overall difference in effectiveness between people 65 and older and younger adults. It also notes that people 75 and older had more hip and pelvic fractures on Wegovy than on placebo in the cardiovascular outcomes trial. People 75 and older, on Wegovy or placebo, also reported more serious side effects than younger adults.

Section 2 of the label gives no dose adjustment for kidney or liver conditions. Section 12.3 reports no clinically significant differences in semaglutide levels with kidney impairment at an eGFR of 30 to under 90 mL/minute, or with mild, moderate, or severe liver impairment (Child-Pugh A to C). That passage does not address an eGFR below 30. It does tell prescribers to monitor kidney function in people who have side effects that could cause fluid loss, especially during dose initiation and escalation.

For people with diabetes, the label says to monitor blood glucose before and during treatment. It says to consider lowering the dose of any insulin or insulin secretagogue when Wegovy starts, because the risk of low blood sugar rises with those drugs. Section 2.2 lists the MASH dose separately, at 2.4 mg with a fallback to 1.7 mg, as covered above.

Wegovy dose and side effects

Higher Wegovy doses bring more of some side effects, and the label's dose tables show it. In the 72-week trials, nausea was reported by 39% of people on 7.2 mg, 35% on 2.4 mg, and 13% on placebo. Skin tingling or burning (dysesthesia) was 22% on 7.2 mg against 6% on 2.4 mg and under 1% on placebo. The slow climb in the schedule exists to reduce the gastrointestinal risk.

In MASH, the label allows a decrease from 2.4 mg to 1.7 mg if 2.4 mg is not tolerated. The only weight-loss trial in the label with a 1.7 mg arm, Study 6, showed a 9.6% mean loss against 13.2% at 2.4 mg.

Is Wegovy dosed in units or clicks?

Wegovy is dosed in milligrams, not units or clicks. The label and the Instructions for Use express every dose in mg, and single-dose pens are preset to one strength, so the pen delivers that dose. The 2.4 mg FlexTouch pen holds four 2.4 mg doses. The label describes no splitting or unit conversion for Wegovy pens. The FlexTouch Instructions for Use describe turning a dose selector until the counter shows 2.4 mg, and they say not to set the dose by counting clicks. Ask your prescriber about any method the label does not describe, including unit counts.

Do teenagers get different Wegovy doses?
